To place your order:

1. Print 2nd page of order form OR write on piece of paper: student name, class, item and size purchased.
2. Make payment via Zelle or Check or Cash.
3. Send in order form and payment back to school with your child in envelope addressed to "PTA / Spirit wear."

Zelle: make payment to "[email protected]." Under MEMO, include student name, class, item & size purchased. *print and submit Zelle confirmation with order form.

Check: make check payable to "PS 221 PTA". Print and submit order form with check in envelope.

Cash: Print and submit order form with check in envelope.
